Yes. Dr. Kathleen Meehan-de la Cruz and Dr. Steve Nguyen are accepting new patient applications at the University and LaSalle locations. And Dr. Nathaniel Eni is currently accepting new patients at UMC Amherstburg Walk-In Clinic & Family Practice.
This service is available to residents of Windsor and the surrounding area. As long as you are a permanent resident of Canada, you are eligible to apply, regardless of where you currently live.

Yes. You can either complete your New Patient Application online or download the New Patient Application Form and submit it in person.

Completed paper application forms can be dropped off at any UMC Family Practice location.

  • Patients arriving late may have their appointment shortened or rescheduled.
  • Appointments are generally limited to one concern per visit. Additional concerns may require a separate appointment.
  • Prescription refill requests may require an appointment and processing time.
  • Forms, notes, and other uninsured services may be subject to fees.
  • The clinic maintains a zero-tolerance policy for abusive, aggressive, threatening, or inappropriate behaviour toward staff, physicians, or other patients.
