Completed paper application forms can be dropped off at any UMC Family Practice location.

Can I use UMC's walk-in clinic while I wait for a family doctor?
Yes. Walk-in care is available at all four UMC locations in Windsor, Riverside, LaSalle and Amherstburg, so you can be seen for minor illnesses and injuries while your application is being reviewed.
Can I choose which doctor I want?
You can select your preferred physician when submitting your application online.
If you’re unsure which physician to choose, our reception team will be happy to assist you when you drop off your completed application form in person.
