At UMC University, our family physicians prioritize long-term care, preventive health, and supportive doctor–patient relationships. New patients are accepted on a first-come, first-served basis, and all applicants must complete a New Patient Application Form.
You can complete your New Patient Application online, or download the application form and submit it in person at HealthFirst Pharmacy. Once received, our team will review your application and contact you with the next steps.
